The forms provided below are for your convenience and can be completed prior to your appointment. If not, please plan to arrive 15 minutes early to your initial appointment to complete the intake form.

• There's no need to complete all the forms; I will specify which one you require. You may fill out the form, save it on your computer, and email it to me. Alternatively, you can print the completed form and bring it to your appointment. If you opt to print the forms and handwrite your responses, please do so legibly, as the spaces are limited.
• If the toolbar for filling in the form is not visible on your computer, consider switching to the EDGE browser if you are using a PC. I cannot guarantee compatibility with Apple Computers and their browsers.

To complete the form on your computer, please adhere to the following instructions. 

YES, typing in the boxes is allowed. I prefer typed forms over handwritten ones for better legibility.

• Click the selected form(s) so that a PDF version fills your screen.
• Find the "T" in a box in the toolbar and select it.
• Then begin filling in the form, using your mouse to click on each entry to type your answers. 
• On the Body map form, select DRAW in the toolbar. Choose any color. Please move the thickness to the middle. Find the areas on the body maps that are of concern. Place your mouse over the area selected and click the mouse. If you make a mistake, that's ok. There is an eraser in the toolbar. Select it, then place your mouse on the area to delete and click your mouse. 
• If you feel you need to highlight an area(s). you may do so with the highlighter option.
• Remember to save EACH document before you leave the screen if you want a copy for yourself.
